On Thursday night inside Rocket Mortgage FieldHouse, the former Knicks lottery pick corralled a loose ball near midcourt with 2.1 seconds remaining in regulation, took two dribbles, and launched a three-pointer that bounced twice on the rim before falling through the net. The Cleveland crowd — and the Cavaliers — went silent. Barrett had forced overtime. And then, in the extra period, he delivered again, keeping Toronto's season alive and forcing a decisive Game 7 on Sunday in Ontario.

The play itself defied reasonable probability. Barrett was not the primary option on the possession. The Raptors ran a set designed to free Scottie Barnes or Immanuel Quickley from the far side of the floor. What materialized instead was chaos: a turnover, a loose ball, and a 24-year-old wing who had averaged 18.3 points across the regular season deciding, in real time, to take the shot anyway. He called his own number. The shot fell. Cleveland, which had led by as many as nine in the fourth quarter, was suddenly in survival mode.

The Cavaliers would survive that night — but barely, and not without consequence to their own trajectory. Donovan Mitchell, the franchise cornerstone Cleveland acquired from Utah in 2022 and rebuilt around, finished with 29 points but shot just 9-of-23 from the field. More concerningly, he appeared to favour his left leg after the halfcourt scramble that preceded Barrett's equaliser. Head coach Kenny Atkinson did not confirm the extent of any injury post-game, stating only that Mitchell would be evaluated before Sunday's tip. Whether the knee that has interrupted each of his last two seasons is again compromised is the single variable that could reframe the entire series.

What Makes This Series Different

The first round of the 2026 playoffs has produced its share of dramatic moments, but few have carried the structural tension of Cleveland-Toronto. The Cavaliers entered the postseason as the Eastern Conference's second seed, a franchise that rebuilt around young talent and smart veteran acquisitions and finished the regular season with the league's fifth-ranked net rating. Toronto, by contrast, spent much of the campaign in the play-in wilderness before a late surge — and a coaching change — propelled the franchise into the seventh seed. The talent differential, on paper, favours Cleveland by a comfortable margin. The on-court product has not reflected that margin.

Part of that is matchup-specific. Cleveland's drop-cover defensive scheme, effective against teams that operate primarily through the post or the elbow, struggles against Toronto's motion offense. The Raptors move the ball quickly, use off-ball screens to free shooters, and do not rely on a single isolation creator — at least not until Barrett seized Thursday's moment for himself. The result has been a series of games decided in the margins: possession by possession, half-court set by half-court set, with neither team establishing the kind of separation that seeds typically project over their lower-ranked counterparts.

The Franchise Stakes

For Toronto, the stakes are straightforward. A franchise that has not advanced past the first round since winning the 2019 championship is playing for the right to extend a season that appeared finished after Game 4. Masai Ujiri, the Raptors' president, traded for Barrett last summer specifically to provide a secondary shot-creation option alongside Barnes. That trade — sending OG Anunoby east and receiving Barrett plus three first-round picks — was scrutinised at the time as a win-now move that sacrificed depth for talent. Thursday night offered the most compelling evidence yet that the gamble may be paying off. Barrett is playing the most important basketball of his career, and he is delivering.

For Cleveland, the calculus is more complicated. This was supposed to be the season the young core — led by Evan Mobley and Darius Garland, with Mitchell as the proven closer — announced itself as a genuine contender in the East. The Boston Celtics, despite their own injury complications, remain the standard. The New York Knicks have reinforced. The Philadelphia 76ers, if Joel Embiid stays on the floor, remain a nightmare matchup. Cleveland needs Mitchell healthy and effective for any of those potential series to become winnable. A knee tweak in a Game 6 does not yet rise to the level of crisis. But in a city that has spent the better part of a decade managing the aftermath of LeBron James's departure, the anxiety around franchise health is not theoretical.

Sunday's Calculus

The line for Game 7 opened with Cleveland as a seven-point favourite, a number that reflects the home court advantage and the differential in regular-season performance. The smart money has been less certain. Toronto has won three of the six games in this series — including two in Cleveland — and has shown a willingness to trade blows with the Cavaliers rather than retreat into conservative half-court sets. The Raptors play fast, they defend with physicality, and they have demonstrated a collective composure in clutch moments that rarely surfaces in a seventh seed.

The question of Mitchell's health may be unresolvable before tip-off. Atkinson will face a decision: give your star player the benefit of the doubt and hope the knee holds, or err toward caution and potentially cede a game at home to a team that has already proven it can win there. Either choice carries risk. The decision will define how the franchise approaches the weeks that follow, regardless of outcome.

What is not in question is the quality of the basketball. Six games have produced a combined average margin of 4.2 points. Neither team has won back-to-back. The series has delivered exactly what the first round rarely does: genuine unpredictability, genuine stakes, and genuine star-making moments. RJ Barrett's improbable three-pointer in Cleveland on Thursday was the latest. Whether it becomes the defining image of this series depends entirely on what happens on Sunday afternoon.
